Balance Adjustments HDL for state unemployment taxes not loading

Summary:

I need to adjust unemployment taxes for several hundred workers in time for quarterly taxes but can't get the HDL to load without errors. I have attached a photo of the HDL I'm currently working with in order to affect the appropriate adjustment to a single worker and expand from there. Has anyone successfully uploaded this HDL before? Any guidance as to where I'm going wrong would be very helpful. Thank you.
