Entity shows or display twice in Web Form POV

Summary:

In Test Environment 26.09.58, we are seeing an issue in Web Form where in when user type in the entity to be used it does shown twice in the display.

This form is designed to show the main hierarchy only, and the entity doesn't have any shared members within the hierarchy.

In Prod Environment 26.08.95, same testing we did and it shows the entity once only.

Anyone experience the same thing in Test Environment?
