Not able to return Questionnaire information from Performance Subject Area

Summary:

I have created a new performance document but am unable to have any data returned from the workforce performance - performance rating real time subject area.

Content (please ensure you mask any confidential information):

The performance document has several questions from questionnaires but no overall rating section. I can view the performance document name, questions etc but when combined in a report nothing is returned.

The generic information can be returned in the other Workforce Performance subject areas but they don't hold the questionnaire information, even if I combine the subject areas the questionnaire information isn't returned.

Any help or guidance would be much appreciated.
