Multiple active workers lost all roles and user accounts became inactive

Hi,

We recently encountered an issue where multiple active workers (not terminated and with active work relationships) were found with inactive user accounts.

During investigation, we observed:

• The affected users no longer had any roles assigned.
• The user accounts were inactive/suspended.
• Re-activating the user accounts alone was not sufficient; roles had to be manually re-added.
• The workers were not leavers and their work relationships remained active.
• We couldn't identify any execution of Send Pending LDAP Requests or Autoprovision Roles for All Users around the time the issue occurred.

Oracle documentation indicates that user accounts without roles can be automatically suspended. However, we are trying to understand what could have caused the roles to be removed from multiple users simultaneously.
