Taxable Benefits HSDL

I'm trying to create a HSDL template to load Private Medical Insurance or Treatment but continue to face the following upload error when testing the data. I have tried: Taxable Benefits, Basic Benefit Information, Benefits, Private Medical Treatment or Insurance, and even the employee assignment. Can someone provide a template that they are using that works or provide some guidance on what content should be in this field? Thanks!
