Creating a Web Report to Export OBLPN Data from Multiple Facilities

Summary:

I am trying to create a web report that will generate an excel (.xlsx) file that contains a few of fields from the CSV that is available when exporting the results of the OBLPNs screen.
I only need a few of the many OBLPN fields, but generating the report is not intuitive at all, and even when trying to generate a report while filtering for only one day of data for one facility, the report either returns records that I didn't intend for it to return, not all of the data I was expecting, or the report engine returns an error pop-up.
