No data in document of record letter from HCM Extract

Summary:

I have followed the instructions to create the document of record letter from HCM Extract.

The document of record letter is create but the mail merged fields are all appearing blank.

If I run the letter as a report then it works ok.

What am I missing? Any thoughts?
