Assessment/Survey Instances "Show Questions grouped by Category"

Hello All!

I am trying to develop a survey.  There are 18 questions in five categories (all five categories are sub-cats in the same category).  In setting up the instance, I select the option "Show Questions grouped by Category" and "Show Category in Header" options.  When I run the survey all of the questions appear on the screen at once, instead of each category on a separate page as the reference manuals suggest.

I have tried several different ways to assemble the root survey:

• Manually select the questions to appear on this assessment.
• Select question category/categories and have questions selected randomly during assessment administration
