HELP!! I created a survey and having problem

I have been waiting all day for support to get back to me and when I called two different times of the day the SR rep hung up on me.  I created a survey using the assessment/survey wizard.  Connected an instance to the survey.  How can I display it on a learning page? I want to give a credit completion once they are done with the survey.  I did set up a credit but I can not map it to a survey.  In the survey questions can I force the user to complete each box before moving on?  I also
