ILT - Feedback form

I created a writable/fillable PDF form. We want to use it as a feedback form after the employee takes a ILT and we want the employee to receive credits when they complete the form.  I have the document in the Resource manager.  Should I create a "course" with a splash page and attach it to the course?  Any suggestions on how I might do this would be great!
