Enrollments for ILT - Self Enrollment

In the past we have been able to create enrolllments that allows the users to have an 'enroll' button for an ILT and it would enroll them as 'nominated.'  The user got a message stating they were 'nominated' and waiting for 'approval.'   Then after an administrator or Instructor changed them to 'approved' the user would get an email stating they were enrolled in the course.  Now I can only find a way to have the enrollment button if I select the box

Automatically approve all users who sign up for this enrollment.  How can a let the user enroll without them being automatically approved?  Am I missing something?  Thanks.
