External Training Dilema

Good moring TKE!

I'm experiencing a dilema when setting up a DO for External Training and am looking for your advice/assistance/comments. 

I've set up my DO on the page as shown below:

HOWEVER, not only is it not showing in the order I have requested in the DO, but the column names won't change AND, I've specifically excluded the Expiration Date but it's still a field in the External Training Details  :

I've attached larger copies of the above photos.  Any help in pointing out what I'm doing wrong would be greatly appreciated!
