Forms/ Self Appraisals

Hi,

Does anyone have any experience of using the Forms feature in Learn?  I can create a work flow via the Form option in the control panel but am having problems with the templates themselves.

If I use an existing sample template, users cannot type in the text areas on the form.  If I try to create a template with the WYSIWYG editor the system just freezes.   If I try creating a form with the HTML editor, it lets me save it but then won't let me set it up as part of the workflow; when I go to Forms I just get an error message "Validation error has occurred:  Please select Template.".
