Want to create a form with 2 workflow approvals

Hi,

I'm looking to create a form (a goal worksheet) for an employee to complete.  I'd then like it to get routed to their supervisor for approval once the employee submits it (to approve the goals their employee has creted).  Once the supervisor approves the form, it will get routed to a group of 4 people (a steering committee here).  I can't seem to find a way to do this.  I know the External Training feature has workflow approvals built in, but this doesn't have anything to do with external training.

I'd then like to include a link on the LearnCenter homepage for the employee to click to complete the form.  I'm assuming the supervisor and then members of the final group will go to the same location to approve them?
