Anyone using Surveys for ILT sessions?

We are attempting to use surveys after an ILT session. We have set up the questions, survey, mapped the instance, turned on ILT post session survey message. The enrollment roster has a post status of completed yet the email doesn't go out and even through adding the "action" column and "review" column for the completed enrollments DO the user has no way to launch the survey.
What are we doing wrong or is there a bug?
