Assessments- Multiple Scores

Summary

Is there a standard report that will give me multiple scores on an Assessment?

Content

Participants have 3 attempts to pass an assessment so I need to see how many times they have failed before sending out reminders. Is there a report to run? I am testing now, just thought the community might have a quick answer for me .
