Bulk Loader session attempt "failed" with no results file

For the attached text file, the first session loaded, but without our custom fields populated, and the rest did not load at all. Each time I have tried, the results said "failed" as you can see from the image, but there is no results file to see what the issue is. Completely different behavior than I have ever seen before.
