I have put together a sample Assessment Report using BI Reporting and a custom RTF layout file.

The custom layout will have a column for each question in an Instance, the row data returns the individual user's responses to those questions. 

The RTF file is based on data elements returned in the associated Assessment Report Design.pdf.  The RTF file will not work unless you create a report using this design.  

Once you have it all working, feel free to modify the report design and RTF to meet your needs.
