Storyline SCORM generates API error upon exit

We've build a new course for end users which mirrors past developments but for some reason the module is generating an API error upon exit. When I review the debug log it appears as if the LMS is recognizing that a user has successfully completed the module's assessment. Their score is captured and there is even a lesson status line with a "completed" value. I'm submitting a service request but was wondering if anyone ever encountered a similar experience. I've uploaded a copy of the debug log with both data model and command stack views.
