Best solution to gather user input answers/ view data?

I am being frequently asked to create materials and forms that a user can fill in their own experiences or answer questions in their own words. What is the best and easiest way to deliver something like this to users that we are able to view the user input answers?
This is starting to be a must-have feature for us and I do not feel like I have found a good solution for this.
What I've tried so far:
Assessments in Learn- This was the only one I was really able to pull data from, but it is not easy to set up and honestly is not visually appealing. Is it possible to add HTML to these?
