Credit Reports - how to get all data in one report

Does anyone have a build on a report that will provide all of the following: credits for courses, user name, user manager, user job title, user location, user company, user group if applicable, date when credit was earned, how many times course was taken before passed

 

I am looking to put as much data into one report as possible while extracting the credits information. So far I am having to use multiple reports to make this work, which is making my 'customers' unhappy.
