Column order with BI Reporting CSV Standard data set

We've received several reports over a number of months where admins have noticed that the selected column order for a BI Report was not correct when selecting CSV output.  We've done some investigation into what was happening and wanted to share that with the community.

The issue is visible only when you select the Standard data set CSV output and not the Full data set option that was introduced in 16C.  The Standard data set outputs use the BI Publisher server to generate the output file, regardless of the selected output format.  What we've noticed is the CSV output handles the placement of custom fields differently than the other formatted output options (ex: RTF, PDF, XLS).
