Cannot Enroll when "Require Supervisor Approval" is Yes for Enrollment

No "Enroll" button is visible

On the Enrollment, the "Require Supervisor Approval" box is checked.

Using the LearnCenter Search DO, the enrollment is found and clicked, but the pop up has no "Enroll" button.

What have we missed in the setup?
