HELP!!!!

I need to pull item ratings and item comments a manager entered in the Competency section in a performance document. 

The report I built last year no longer works - it returns blank ratings.  We are on R10.  I also tried the SQL below that I found on Customer Connect, but it also returns blank ratings.  PLEASE HELP!  This is pretty urgent :-(
