Goal Report not adding up.

One report says they have 4 goals another says they have 1

Hi all,

I am having some confusion on getting my report to work correctly. 

I run a report i build that says per this goal plan name and user is active then show me how many people have 1 goals.

This report is showing 2 individuals that have more than 1 goal but have 4. I have validated this. 

Why would this be occuring? Attached is my report
