Duplicate Rows for a report trying to capture each employees self and manager evals

I'm trying to create one report which will capture an employee's self eval and performance review status'. If it worked perfectly an employee would end up with only two rows; one for the self eval and one for the actual performance review. But, for whatever reason I'm getting 4 or even 6 rows. Any idea what I'm doing wrong?
Excel output attached, as well the XML on a separate tab.
