Creating a survey/assessment and attaching to an Enrollment

Hi there,

Here's what we would like to do:

Create a suvey/assessment that is a Guest Profile questionnaire - the purpose would be that once someone Enrolls in an ILT session they would be prompted to complete this (unscored) questionnaire regarding travel accommodations (lodging, air, travel preferences). I've created the Assessment and Instance and mapped the Instance to the Enrollment but can't see a way for a User to access the Assessment. 

I've seen some older posts about this but didn't find a solution so wanted to check to see if anything new has developed. The only other idea I have is to create a form externally (PDF probably), load to the Resource Manager, and attach it to an Enrollment. This isn't ideal because then the User would have to complete, save, and attach to an email and send to our event coordinator. I hope I'm missing something as I don't have much experience with Assessments/Instances. Anyway, any ideas are welcome! Thanks for reading!
