Information Template Customization

Summary

In an Information Template, if I select 'YES' to one question then the other question should pop up and if I select 'NO', then those fields should not popup

1) We have enabled Information Template for certain categories on Requistion Line Level, but we have requirement to have questionnaire in dynamic way.That is if I select 'YES' to one question then the other question should pop up and if I select 'NO', then those fields should not popup.
