Auto Provisioning of security roles not providing the pre-req roles

One employee in my org just got promoted to a supervisory position. This employee should have three auto provisioned roles but doesn't.

I was comparing this new supervisor's roles to his counterparts' and noticed that he doesn't have the same three auto provisioned roles.  They all share the same job title/class/grade/etc. 

Under person management, I compared this new supervisor's roles with another supervisor.  Both employees have the same boxes checked and yet, under security, they both don't have the same three security roles. 

They both should have: Line Manager, XYZ Custom Line Manager & Employee
