How to link a Dependent with their Dependent Care Physician

I am trying to pull a report of employee and dependent care physician after Open Enrollment. If an employee has more than one dependent I am getting 2 rows for each. Is there a way to only return the one that's linked to the dependent? 

This field is not under the Dependent subject area but wondering if it actually is a data field. How could I find the name of that field?
