Some employees receiving insufficient balance notice even with a sufficeint balance for PTO

With our November patch we are finding that some employees, not global, and not on one particular plan group (i.e. we have found this on our PTO-Management, PTO Non-Exempt and PTO-Exempt plans) are receiving an insufficient balance error when trying to enter a PTO request. The balance is sufficient and in some cases even the administrator cannot override this, which the plan is designed to allow. We are also receiving this error when trying to run a manual accrual on the some folks. How can an accrual of PTO trigger an insufficient balance error??
