We are looking for a place in HCM to conduct and track exit interviews. I would like to try using the configurable form within the checklist to capture employees’ feedback from an exit interview. This will need to be a combined effort between the employee and their HR representative. The manager will also need to have visibility to this information.

I am looking for a worked example or instructions on how to build this configurable form to use in the checklist. It would be helpful to see what this could look like and how it can interact with the checklist.
