My Process Application Won't Show Up On Workspace as an Application to Initiate

Summary

I published and activated my process app. I can't initiate a request from the Workspace.

Content

I built process application containing one process.  I made sure I was added a user for the swim lane role.  I published it.  I activated it.  I can run it in player successfully.  I can run it successfully in Test.    However, I can't see my process application as one I can Initiate Request.  See screen shot.  How do I make it appear there?
