OIC Integration is getting called three times via PCS process

Hi Experts,

 I have created a Simple Notification REST API in OIC which sends the notification only. I'm trying to call this Notification REST API via the PCS Integration. This is how the PCS process starts:

• Open a Form in which we enter some information
• Goes to the Human where he Approves the form
• After this activity I call the Notification REST API.

This notification REST API is getting called three times. Couldn't figure out why this is called three times. 

Can someone help me to resolve the issue
