New Starter Declaration (UK) - Access Denied

Hello,

We are trying to use the New Stater Declaration for Employees - the icon was not showing but we have followed the steps below and the button now shows, however, when the employee clicks on it they are presented with access denied, are any other privileges needed? - Anyone else using this quick action?
