Benefits Pending Action Item Report - Incorrect Data

I've created an OTBI to report on Pending Benefit Action Items, but it pulls in employees that have already completed them.Content
I have created the attached analysis for pending benefit action items (anything that doesn't have a completion date), but it is pulling in items that have already been completed. I've attached screenshots. Does anyone else have experience with this or another example of a report.
