I am attempting to follow the instructions in the link below in order to Manage Data Access for Users in MS Excel:

The issue occurs at steps 7 and 8 in the Assigning Data Access to Users Using a Spreadsheet section, when clicking the Authorize Data Access to export search results into Excel. My screen shows a template with no headers or dropdown selections, when I would expect to see the attached image.
