Learning Course completed by both employee and instructor

We have a scenario where a course has two parts a theory and a practical evaluation. The theory part is completed by the employee ( via assessments). If they pass the theory , the trainer will conduct a practical assessment ( also fills up a questionnaire). If both the activities are completed the employee's competency needs to be updated as an outcome of the course.

The requirement is to have the trainer also fill up the questionnaire and not just mark the activity as complete.

Looking for suggestions and possible solutions to cater to this requirement and if anyone has come across similar requirements before.
