Issues updating Chart of Accounts

We are updating our CofA as of Jan 2020, but are having problems getting it to jive

I have created a number of new Ledger Code Value which we call Departments and Accounts (Parents and Children) for an update of our Chart of Accounts as of Jan 2020.  I have created new Departments and Accounts before, and put them into the Hierarchies, and Published them before, without issue.  But this time there are dozens and dozens of new Accounts and Departments, including Parents, and I don't think I understand the rules of what I can and can't do, so I'm stuck.
