Upload Images custom tab is blank

Our custom tab is blank since a few weeks. This tab should upload images which later are used in answers

Hi!

In our ORN instance we have modified our answers workspace (somebody before us in fact, a long time ago, but we are not sure who and when) to include a new tab to upload images that are going to be used in the answers.

For some reason it stopped working a few weeks ago and it now shows a blank space.

Has anyone had the same issue or similar? Does anybody know which can be the cause for this issue?
