Welcome email not being triggered

We have an issue whereby Welcome emails are not being triggered for a user when they get synchronized from Fusion. All other notifications go out as expected (i.e. account locked, password rest etc.). Also, when trying to 'resend invitation' we get the following message:
You are trying to send an activation email notification to a user who is already verified
Is this a set-up issue or something else?
