Responsive Convert Pending Worker Screen is Blank

When a user goes to use the convert option in RUX the page comes up blank

Hi,

Our users have experienced an issue where when they try to complete the convert action via the pending workers screen they are returned with a blank screen (see screenshot) and not able to edit or progress with the transaction. 

Thought this could be a caching issue but we have cleared cache and tried numerous browsers and had no luck. This screen is working in our development environment which leads me to believe this could be a bug in our test environment. I am going to raise an SR but thought I would also post here to see if anyone has had a similar issue.
