Error messages when performing "Convert Pending Worker"

Hi,
When users try to convert a pending Worker to New Hire - they get an error message when press Submit.
It happens when using the "New Person Dashboard".
I have two samples of error messages that users got in different occasions(see also attachments):
1. ADF-FACES-60097: for more information , please see the server error log for an entry beginning with ADF_FACES_60096 Server Exception during PPR, #65
2. an error occurred while processing the data. cancel and reenter the transaction

Please advice me about finding a solution because it happens a lot of times.
