Non Catalog item DFF crashing workflow and notifications

Hello, all
Recently I've created the DFF list for Learning request non catalog item: WLF_LEARNING_REQUEST_DFF
Though after this the workflow is no longer going through all the steps pre set. also not sending emails nor displaying the buttons of approve and reject in bell notifications, also not displaying any info in in the BPM worklist task.

Am I missing a configuration?
does Anyone know what can i do to workaround this?

Note: when I hide the context value and therefore all FFs, it appear to be working correctly.
