Inconsistent SCORM completion issues

I have posted about this around a year ago, but we still have not got to the bottom of the issue so wanted to see if there were any fresh ideas. We have a very sporadic and inconsistant with eLearning, where a user completes the package and clicks the save and close button, but the course remains in progress. This is one of the most frustrating issues we have had with the module as it is very hard to diagnose and remedy.
Example:
We have a course called SmartCash which we have pushed out to our food business. We have 4000 completions so far but around 50 users contacting our service centre to say that they have completed the module but it is still showing as in progress and not completed.
