Centerpoint Issue: Resignations not working

Resignations cannot be submitted. If an employee tries to submit a resignation (Me --> Show More --> Employment / Resignation) a myriad of issues occur.

Issue Description:
Resignations cannot be submitted. If an employee tries to submit a resignation (Me --> Show More --> Employment / Resignation) a myriad of issues occur.
1. If they try to submit a resignation for a future date (most common), the action list does not populate any values (screenshot attached) AND they get an error message when they click Next on the When & Why screen (screenshot also attached)
2. If they try to submit a resignation for the same date (rare but possible), they get an error message (screenshot attached) when clicking Next in the When & Why screen
