Cannot create department using Hdl spreadsheet

Hello All,

I will appreciate your support in the issue below: We have filled the data related to a Department. After clicking on Upload, we get the error below, can you please advise why?

Moreover, when filling the Date Set Name, I got the an error as well. PFA document with screenshots
