User has two required activities for an offering that only has one activity

Experiencing an issue where a user somehow has two activities to complete under an offering that only has one activity attached to it. When the user returns to the course to resume/complete they receive a message they have an active enrollment but then it will not allow them to launch the content and I suspect is has something to do with the double activity on the offering (see attached). I can't figure out how to remove the second activity so there's only one attached to the offering for this user.
