Smartview Manage Dimension unable to create new member (An object already exists)

Sorry if this is a duplicate since my previous post was somehow going through administrator approval; probably I didn't use the new post template well :(

I was kind of casually adding some new members through SmartView Manage Dimension. And those members, as shown with asterisks below, somehow staying on the form but not really being added in the system. When I wanted to assign them to different parent, I was prompted the object already exists.

Yet when I did a search, it's actually not there.

I did DB refresh and even resetService but this issue was still here. How can this be fixed?
