WF - Checklist real time - Rensponse answer not displaying Date values

Workforce management - Checklist Real Time -- Added Question Text and Response Answer columns. We have 4 questions - 2 text and 2 Date type.

When ran the analysis, only showing 2 text column values, but not showing the date values of the response.

Can anyone please provide the SQL query to create custom BIP report.
